There was an initial release of NPC's but they've been out of the game for quite some time now. There are Meta-events that make it seem like there are people (gunshots/choppers flying by) Link to comment

Fallout New Vegas with hardcore mode adds survival aspects, 7 days to die is also a good one. Rust is somewhat survival but not that much. Link to comment
